Minnesota care providers. A shared future.
Minnesota PCA/CFSS & 245D providers

You built the business.
You don’t have to carry it alone.

We’re North Star—experienced operators building lasting relationships with Minnesota PCA/CFSS and 245D provider owners. We bring people, capital, and hands-on commitment to the businesses we partner with. It starts with a personal introduction.

A shared ownership partnership. Our investment goes into growing your business.

A personal introduction

You may have received
a card. A call.
Or met one of us.

We reach out personally because a lasting partnership starts with a real relationship. We want you to know who we are—and we take the time to understand what matters to you.

You don’t need a sale price, a growth plan, or a decision about your future to have a conversation with us.

We show up. We listen. We follow through.

We spend time with owners, keep our commitments, and speak honestly about where we can help. When there is mutual interest, we work through the business together and develop a specific ownership and operating partnership.

What North Star brings

The bridge between
where you are
and what comes next.

Your business represents effort, relationships, and personal commitment. We respect that. We bring resources and hands-on execution to help you move forward—whether the priority is growth, stronger operations, or a different role for you.

01 / INVEST IN GROWTH

Put resources behind revenue.

Fund the marketing, outreach, and people needed to build referral relationships. Connect that demand to organized intake and the capacity to serve clients well.

02 / RUN THE BUSINESS

An operating team beside you.

Take on agreed management responsibilities, support your people, and strengthen the systems behind service delivery. Give the business the attention it needs while changing how much you carry personally.

03 / SHARE THE VALUE

We commit. We build. We share.

Our capital, people, and operating commitment come in return for an agreed share of ownership. You retain a stake and participate in future value under the partnership terms. We define what each side contributes before we begin.

Our world is your world

Experience running
Minnesota care businesses.

We connect with owners at different stages—from new agencies to established providers generating $200,000–$3 million in monthly revenue. Some want to grow. Others want an operating team to take responsibility, or a path forward while a full sale remains out of reach. We take the time to understand the person behind each business.

PCA / CFSS · 20+ years operating experience

We know what it means
to be the person everyone calls.

Your staff need answers. Families rely on you. Decisions follow you home. When you’ve built an agency, the responsibility can stay with you long after the working day ends.

We’ve spent more than 20 years operating in this field. That experience shapes how we approach owners: with respect for what you carry, and the ability to discuss taking real responsibility alongside you.

245D · 5+ years operating experience

You see the business.
And every person behind it.

There are people relying on the services you provide, a team looking to you for direction, and relationships you’ve worked hard to build. Any decision about the future has to account for all of them.

Our experience running 245D services gives us a place to start that conversation. We want to understand what you want to preserve, what you want to change, and where we can take responsibility together.

How we work together

Shared ambition.
Clear responsibility.
Honest conversations.

A real partnership needs trust in both directions. We earn yours through our work, transparency, and accountability—and agree on the authority our team needs to deliver.

A PLAN WE BOTH UNDERSTAND

Agree before we begin.

Define the investment, ownership, operating responsibilities, goals, and your role. Everyone knows what they are committing to.

ROOM TO EXECUTE

Give the team a clear remit.

We work within agreed decision-making authority, with reporting and oversight built in. Clear roles let the business move forward.

FOLLOW-THROUGH THAT MATTERS

Say what’s working. Fix what isn’t.

Review progress together, discuss setbacks openly, and agree on corrections. Challenges get a clear response and a responsible person.

Support behind the growth

Grow with
greater confidence.

With an experienced partner alongside you, everyday operations get the attention they deserve. We help identify concerns and put practical improvements in place as your business grows.

Documentation gaps, billing inconsistencies, and unresolved service complaints are among the issues that deserve attention. These and other operational concerns may raise compliance questions or signal potential fraud—even when an owner believes the business is running properly.

We work with you to understand what needs attention, strengthen processes, and support your team in addressing the underlying issues. An indicator calls for review; it is not, by itself, proof of wrongdoing.

Talk through your agency’s next steps
From connection to commitment

The relationship comes first.
The plan is built together.

Our purpose is to build and operate care businesses through shared ownership. The right arrangement begins with understanding each other—not choosing a package.

One relationship at a time

First, a real introduction.

A conversation, time together, and a chance to understand each other beyond the numbers.

Then, a closer look.

When we’re both ready, we discuss the business, where we can contribute, and what you want your role to be.

Together, a commitment.

Agree on investment, ownership, and operating authority. Put the people and resources to work, with clear accountability.

Owner questions

Clarity before
commitment.

Why has North Star contacted me?

We introduce ourselves to Minnesota provider owners to build relationships and explore whether we could work together. We’re interested in the person behind the business. An introduction is a conversation, not a commitment to a deal.

What does a partnership mean for my ownership?

North Star commits capital, people, and operating work to your business in return for an agreed equity stake. You retain an agreed share. Before you commit, we define ownership, investment, decision-making, and responsibilities together so you can assess the full exchange.

Does your investment go to me or into the business?

Our model directs capital into the business: people, marketing, systems, and operating improvements. It is not an upfront payment to purchase your existing stake. North Star’s ownership is agreed in return for its investment and operating commitment.

How do I participate in a future sale?

You retain an agreed ownership stake. If the business is sold, your participation follows the ownership and sale terms in our agreement. Our focus is building the business over time; a sale date, sale price, or return is not guaranteed.

What if my agency needs operational help first?

We start by understanding your agency and where support would make the greatest difference. Together, we identify priorities and agree on an operating plan. Our work addresses the underlying issues; it does not erase past liabilities or guarantee regulatory outcomes.

Can your team take over daily operations?

Yes, where that is the agreed scope of the partnership. We define operating responsibilities, decision-making, the transition plan, and your role before starting. Owners can remain involved or arrange to step back from day-to-day management.

Do you only work with smaller providers?

No. We work with growing agencies and established providers, including businesses generating $200,000–$3 million in monthly revenue. A partnership can focus on operating an existing business as well as growing it.

Are you building to hold or building to sell?

We build for long-term ownership, with the option of a future sale to private equity or another buyer. The goal is to create a stronger, more valuable business first. Timing and exit decisions follow the partnership agreement.

Which providers do you partner with?

Our focus is Minnesota PCA/CFSS and 245D in-home providers seeking growth, an operating team, or a path to step back while retaining an agreed ownership stake.

A first hello. Or the next conversation.

Good to meet you.
Let’s keep in touch.

If someone from North Star has reached out, you can reconnect here. If we haven’t met yet, introduce yourself. There’s no need to have everything figured out.
